The packet listed below is an online fillable form and is available for emailing back to us as an attachment to info@cnmhealth.com.

You must first click on the ‘First Office Visit Patient Packet‘ link below to download it to your computer.  Then when your browser opens the packet you must ‘Save As‘ under the File menu to a destination that you can access.   Open the packet from where you saved the file and it will open using Adobe Reader. If you do not have Adobe Reader on your computer then please click the Get Adobe Reader link below and follow the instructions.  Once you have completed the document you can ‘Save As’ it from the File menu.   Please then open your email application and attach this form.  Please type Intake Form in the Subject field.

If you do not feel comfortable with sending your confidential information via email you can either fax the form to (650) 323-2139 or mail it to our physical address at Cypress Natural Medicine; 359 Middlefield Rd; Palo Alto, CA 94301.  Please be aware that is important that we receive your intake paperwork at least 24 hours prior to your initial visit in order to maximize our time together.  It is also helpful to bring, fax, or email any recent laboratory results that you have had done by other doctors.  If you have online access to your previous lab results please bring your user ID and password so the results can be reviewed during the visit.  If you do not have copies of your lab results we can request them after your intial appointment is complete.
